Biling and Insurance


PHYSICIANS MEDICAL CENTER BILLING AND INSURANCE INFORMATION

We are pleased that you have chosen Physicians Medical Center for your healthcare needs. In order to provide you with the best service we can, we have the following policies regarding our services:

  1. We accept most all types of insurance including: Medicare, Medicaid, United Healthcare, Blue Cross Blue Shield, Culinary, Sierra Health, Aetna, Cigna, Anthem and many others.
  2. For your convenience the following methods of payment are accepted:
    • Cash / Cashiers Check / Money Order
    • Check (with two forms of identification)
    • Visa, Master Card, American Express and Discover Card
  3. As a courtesy to you, we will bill your insurance company. If your insurance does not settle within 60 days or denies your claim, you are responsible for payment in full.
  4. All co-pays, co-insurance and deductibles are due in full at the time of service.
  5. You are responsible for current insurance and demographic information. If your insurance, address or telephone number changes, it is your responsibility to notify us immediately.
  6. We accept assignment for Medicare.  You are responsible for the annual deductible and all amounts not covered by Medicare. If you have supplemental insurance, we will also bill that insurance; however, you are still responsible if the second insurance has co-pays or deductibles.
  7. If you are covered by Medicaid, you must have a valid card with you in order to be seen.  If you do not have a valid card, payment in full is required at the time of service.
  8. A fee will be charged for broken appointments or appointments cancelled without a 24 hour notice.  Collection fees will be assessed for returned checks and non-sufficient funds.
  9. We must emphasize that as medical care providers, our relationship is with you, not you insurance company.  While filing insurance claims is a courtesy we extend to our patients, all charges are your responsibility from the date the services are rendered.
